As a Social Prescriber my job role consists of -assessing what support people need,  what matters to them!!  such as learning new skills, making new friends, finding employment, filling out forms, physical activities, looking at what community groups offer and how I can signpost patients to them also any charities that could support- so I refer to non- clinical services such as Adult social care.

This is an approach that connects people to activities, groups and services in their community to meet the practical, social and emotional needs that effect their health and wellbeing.

If you feel that you would benefit from seeing Deborah please speak to one of the Doctors or the Practice Nurse and they will refer you.
